Family Counseling Clinic
(Outpatient Mental Health Counseling)
HFC’s Family Counseling Clinic offers outpatient mental health services for adults (18+) to support individuals with: Daily well-being; Family challenges; Anxiety; Grief and traumatic loss; Mood disorders; Post-traumatic stress disorders (PTSD); Relationship issues; Severe and persistent mental illness. Master’s-level and licensed Clinicians provide a wide array of therapeutic and behavioral health services utilizing evidence-based practices. The Outpatient Program is fully licensed by the State of NJ, Department of Health.
Services include, but are not limited to:
- Comprehensive biopsychosocial assessment
- Individual, group, couples, and family psychotherapy
- Referral and linkage
- Medication monitoring/prescribing provided in conjunction with psychotherapy

Pediatric Asthma Prevention Program (PAPP)
Our Pediatric Asthma Prevention Program (PAPP) is a home-based multi-component initiative that promotes the well-being of children with asthma and their families through coordinated service delivery for families with children between the ages of 0-18 who are in need of education and interventions to reduce their child’s risk of developing asthma and improve their health and wellness outcomes.
Services include, but are not limited to:
- Case management
- In-home environment testing
- Educational workshops
- Asthma treatment plans
- Peak flow meter test
- Linkage to healthcare providers

Healthy Families Program
The Healthy Families Program is designed to promote health awareness, improve health outcomes, and strengthen families. Healthy Families accomplishes this through a multi-intervention approach to case management that prioritizes community education and connection to health and social service resources.
Services include, but are not limited to:
- Family Nutrition Planning
- Case Management
- Education on the social determinants of health
- Cooking Workshops
- Linkages/Referrals
- Health Insurance Application Assistance
