Job Description: The Mental Health Therapist provides high‑quality clinical services to adult clients, conducting assessments, therapy, and crisis intervention. This role includes diagnostic evaluation, treatment planning, ongoing counseling, and referral services in alignment with HFC’s mission to empower and support diverse communities.
Duties and Responsibilities:
Conduct comprehensive bio‑psychosocial evaluations and intake assessments for adult clients
Use interviews, observation, and other assessment tools to identify client strengths, challenges, and needs
Provide individual and/or group therapy sessions, applying evidence‑based interventions
Administer and interpret psychometric measures, including mental status exams
Diagnose client conditions using ICD‑10 criteria
Develop, implement, and regularly update treatment/recovery plans
Maintain and document clinical notes, progress reports, and discharge summaries per regulatory and funding guidelines
Offer crisis intervention and make referrals to higher levels of care when needed
Monitor missed appointments, conduct outreach, and follow up with clients
Submit utilization reports, court or external reports (if required), and timely statistical/clinical data
Participate in supervision, clinical meetings, and collaborative team development
Uphold compliance with funder, Medicaid, agency, licensing, and ethical standards
Build and maintain professional relationships with community agencies and partners
Qualifications
Master’s degree in social work, psychology, counseling, or a related field
Valid professional license in NJ (e.g. LSW, LCSW, LPC, LMFT, LMHC)
Minimum 2 years of clinical counseling experience
Fluent in English and Spanish (required)
Strong computer skills and comfort with Electronic Health Records (e.g. AdvancedMD)
Ability to respond quickly, foster trust, and engage respectfully with clients from diverse backgrounds

Strong interpersonal, communication, and teamwork skills
Ability to manage a caseload independently while collaborating with staff

Domestic and Sexual Violence Case Manager

Job Description: The DV & SV Case Manager provides compassionate, trauma‑informed support and case management to individuals who have experienced domestic or sexual violence. This role works collaboratively with clients to develop safety plans, connect them to resources, provide counseling, and guide them toward healing and stability. The ideal candidate is bilingual (English/Spanish), client‑centered, and committed to empowerment and inclusion.
Duties and Responsibilities:
Conduct intake interviews, assess client needs, and gather client histories
Develop individual care and safety plans with clients, grounded in best practices and client goals
Offer individual and group counseling focused on empowerment, healing, understanding power & control dynamics
Coordinate referrals and collaborative services with mental health professionals, social service agencies, health providers, etc.
Monitor, review, and evaluate client progress relative to goals and safety plans
Maintain accurate and up‑to‑date documentation including case notes, reports, client histories, and service delivery records
Conduct outreach to raise awareness of DV & SV services and connect with potential clients
Participate in case conferences, staff meetings, and cross‑team coordination
Uphold confidentiality, ethical standards, and compliance with funders, licensing, and organizational policies
Qualifications
Bachelor’s degree in social work, human services, psychology, or a related field (required)
Bilingual in English and Spanish (spoken and written) (required)
Experience in case management, counseling, or work with trauma, domestic or sexual violence (preferred)
Deep understanding of intervention strategies, safety planning, and trauma‑informed practices
Strong active listening skills, outreach ability, and group facilitation experience
Ability to work independently and collaboratively in a team environment
Familiarity with local community resources and social service networks
